Accessing Birth Certificates

Access to birth records less than 100 years old is restricted in Connecticut. Pursuant to CT general statues, the following are the only persons who may have access to or be permitted to examine the original or any copy of a birth certificate or birth record – no exceptions will be made:

  • The person whose birth is recorded, if over eighteen years of age
  • Such person's children, grandchildren, spouse, parent, guardian or grandparent
  • Attorneys-at-law and title examiners representing such person or such person's parent, guardian, child or surviving spouse
  • Members of genealogical societies incorporated or authorized by the secretary of the state to do business or conduct affairs in this state
  • Agents of a state of federal agency as approved by the department

Any eligible party requesting a copy of a birth certificate must provide verification of the relationship entitling them to a copy of the birth certificate. Additionally, the requester must submit the following documentation (only these documents are allowed as proof of identification):


Requesting a Copy

Telephone verification of any information concerning the birth record is prohibited. The charge is $20.00 for a copy of a large form birth certificate and $15.00 for a copy of a short form birth certificate. Cash, money orders, or cashier's checks are accepted; no personal checks. Money Orders and Cashier's Checks should be made out to "City of Waterbury, Vital Statistics." There is an additional transaction fee for using a credit or debit card.

Please note: Short form birth certificates are not accepted by some agencies. All birth certificate purchases are final: short form birth certificates cannot be swapped for the large form.

Requests for Copies of Original Birth Certificate of an Adopted Person

An adopted person who is at least 18 years old or the adopted person’s adult child or grandchild may use the form below to request the original birth certificate of the adopted person. All other persons seeking to obtain a copy of the adopted person’s original birth certificate must obtain a court order.

Original records of adopted persons may be stored off-site and are not immediately available. There is a $65 fee. Money orders or cashier's checks are accepted; no personal checks. Do not mail cash. Money Orders and Cashier's Checks should be made out to "City of Waterbury, Vital Statistics."
