In accordance with the City of Waterbury Ethics and Conflict of Interest Ordinance that was approved by the Board of Aldermen in August 2003, all persons or business entities doing business with the City of Waterbury in excess of $1,000 in a calendar year must file a financial disclosure form.

Complete the 2024 Annual Financial Disclosure Form by:

1. Include the vendor number noted on the top of your letter.
2. List open purchase orders or contracts with the City. If none, check the box.
3. Name City of Waterbury officials or employees who have a financial interest in your business. If none, check the box.
4. Sign the certification page.
5. Return the signed form no later than June 1, 2025
