The City of Waterbury is pleased to announce that we are offering a Dental plan option to our Medicare Supplement Plan members. Please look through the materials below and see if this is the right choice for you.

Important: If you are going to enroll in the DHMO, you must complete a Voluntary Dental Special Election / Waiver Form and include the dentist's ID number. Please read the materials carefully and focus on the FAQ's because we have provided the answers to most questions in this document.
