City of Waterbury Employees

Open Enrollment is May 15th through June 15th.

During open enrollment, you may change medical coverage, add or drop coverage for your eligible family members, or enroll yourself if you previously waived coverage. Any changes you make are effective July 1, 2012 for 12 month employees and September 1, 2012 for 10 month employees (including Teachers and School Administrators) unless you have a qualifying status change.

Plan Changes for Prescription Coverage: In December, a special enrollment took place for the changes to our POS and POE prescription plans. The POS plan now has the same prescription coverage as Century Preferred. After the $1,000 cap is met, you may submit claims for reimbursement through Anthem. POE prescription coverage now has no cap.

SEIU – Contract Changes Benefits: Effective July 1, 2012 there will be changes to two SEIU Collective Bargaining Agreements:
Classroom Assistants Bargaining Unit- Copayments and deductibles in each health plan have increased slightly as well as the copayments in the pharmacy plans. Refer to the Plan Comparisons or the Collective Bargaining Agreement for specific details.
Secretarial & Clerical Bargaining Unit- Copayments and deductibles in each health plan have increased slightly as well as the copayments in the pharmacy plans. In addition, the POS plan has been eliminated as a choice for coverage. Century Preferred and the Bluecare POE are still offered. If you are enrolled in the Bluecare POS Plan, you must make an election to change coverage during open enrollment. Refer to the Plan Comparisons or the Collective Bargaining Agreement for specific details.

Make Sure You Cover Only Eligible Dependents:
Dependents eligible for coverage include your legally-married spouse or civil union partner and your children, including stepchildren and adopted children. Be sure that you are not covering a former spouse from whom you are legally divorced, any stepchildren that belong to your former spouse, or any dependent children who are eligible for coverage with their own employer.

Submitting Changes/Additions: Effective immediately, when submitting an Anthem Change form with dependents, an affidavit must also be presented. This affidavit asks you to validate that the dependents you cover are currently eligible.

Update your Beneficiaries (Life Insurance & Pension): Although you can designate a beneficiary at any time, it is a good idea to periodically review and update your life and pension designations. The City provides basic life insurance coverage for eligible employees. Beneficiary forms are available on the City’s website under Pension and Benefits. Employees who contribute to the City’s Retirement System should designate beneficiaries for this plan as well. Beneficiary forms are also available on the City’s website. Teachers and School administrators earn their pension through the State of Connecticut. Forms for state plans are available on their website.
