A pediatrician and a nurse from StayWell Health Center will be available to provide sports physical examinations from 9:00 a.m.-12:00 noon.

• September 3 at North End Middle School
• September 6 at Wallace Middle School
• September 9 at West Side Middle School

Students must bring the completed 1) Enrollment Permission form, 2) CT Health Assessment form and
3) a copy of their insurance card to register for an appointment. StayWell accepts the HUSKY/Title 19,
Medicaid, and the Charter Oak Health plans. Several of the major private edical insurance plans are also
accepted for medical services. We can also help uninsured students, who are not eligible for the
HUSKY/Medicaid plan, get a sports physical examination.
